Health policy means more than governance and funding of universal health care. It aims to improve population health and health services. This book analyses institutions, standards, players, and employment of resources with a view to their actual and potential contribution to health policy understood as social management of health risks before and after the event – i.e. to prevention and health care. Combining approaches from sociology, political science, and health sciences this modern and systematic treatment covers various aspects of the issues involved with special regard to efficiency and equality of opportunity. Topics include:

• History and types of health policy
• Prevention and health promotion
• Health care (out-patient and in-patient care, supply and availability of pharmaceuticals and medicines)
• Nursing and care

Time and again, during its long history, the sociology of inequality has been confronted with a number of theoretical problems which still remain mostly unsolved. These problems can be reduced to two basic questions: (1) How does the alignment between "objective" living conditions and "subjective" life styles come about? (2) What are the rules behind the representation of social inequality, are these representations mere "reflections", "discoursive constructions" or do they follow social rules of their own?

Four different answers to these questions are introduced and discussed: Norbert Elias' established-outsider-figuration, Reinhard Kreckel's centre-periphery-metaphor, Pierre Bourdieu's habitus-field-theory and the system-theoretical distinction between inclusion and exclusion. Each of these answers has its advantages and limitations. The latter incude the fact that they can only offer partial solutions. The task facing the sociology of inequality therefore is to develop auxiliary concepts (Bourdieu) from already existing theories in order to advance practical research.

 

Although no sample of homosexual men can be considered representative, given the hidden nature of the population, the national survey effectively reaches men active in the gay subculture, while also including subgroups of men who are not gay identified and have a weaker connection to gay institutions. The results show that young gay men, men of lower social economic status, certain groups of immigrants, men with a large number of sexual partners, men from the club, party and leather scenes, HIV positive men, and men in steady partnerships are most in need of prevention interventions.

In spite of various overlappings, reception and most of all discussion of the results of social scientists' evaluation of health policy continues along the lines prescribed by the disciplines concerned - that is, sociology, political science, health economy, and  health sciences. Drawing on the knowledge supplied by neighbouring disciplines is rather an exception than the rule. This practice does not only impede the development of a sound theory but also obstructs necessary changes to improve health policy.

One of the objectives of this manual is to help overcome these deficiencies. It presents insights and explanatory models from the various disciplines dealing with health policy and aims at tying these different perspectives together in order to promote present and future health professionals' orientation towards a rational and humane management of health risks before and after their occurrence.

 

Hospital services influence the life and health of some 15 million people annually and the general reliability of hospitals is a "public good". It is therefore important that health policy anticipates possible dangers to this good.

As the debate on the reform of hospital funding is largely reduced to economistic considerations and governance techniques, this study presents the results of a project examining the range of the problem from a health care perspective with the help of interviews conducted during the years 1996 to 1999.  Central to these findings -next to an improvement in process organization and attempts at quality management - is an economization of thought and action on the part of doctors as well as care and nursing personnel. Economization does not mean 'efficiency', but a tendency to distort orientation towards service by economic considerations based on real or supposed economic necessities. There is a growing risk for patients that the aims of service may be overcome by economic targets. An understanding of these interrelations is fundamental for a possible correction of health policy.

 

Offering medical, nursing, public health, sociological, psychological and social work perspectives on the German response to AIDS, this book provides a valuable source of reference for researchers, teachers and professionals working in AIDS prevention and care.

 

The author shows that the " Western" approach did not succeed in reflecting the social realities of its new target group, completely neglecting the GDR past and all experience connected with it. This disregard includes all attitudes of gay men towards AIDS and risk prevention as well as established styles of socializing, the orientation toward life in general and specific structures of communication. The study, therefore, presents detailed suggestions for the improvement of these prevention strategies.
 

While there has been some progress in focussing nutritional training and education on health promotion these endeavours may be and need to be intensified. Analyses of nutrition policies and its effects on public health, discussion of new concepts of public health, integration into training and teaching in Europe and requirements for new professional qualifications - these are the topics dealt with in this volume by renowned authors from the field of nutrition and public health.
 

This annotated bibliography comprises some 250 titles published during the last few years in German and English in the field of nutritional, social and health sciences. It presents a comprehensive overview of this multifaceted problem in order to facilitate its study and research. The disc accompanying this volume can be read and used by almost all  widely spread programmes and provides for additional entries and further inquiry.

Giving a critical balance of health promotion activities past and present from a legal, economic, political, and social medical perspective, the authors point out possible perspectives and development tendencies. They show what value those involved in this field - science, employers, trade unions, health and accident insurances -, judging from their own experience, attribute to health promotion, what they consider to be their future tasks and what activities they plan for themselves.

The authors examine what particular kinds of workload and workload constellations are to be held responsible for the development and chronification of the disease. They discuss models of prevention which suggest an improvement of work-site social relations and behavioral training courses. By empirical work-site studies the authors look into the reasons for the difference between what is scientifically sound and what can be put into practice focusing on the question to what extent the behavior and interests of different stakeholders show potential starting points for an innovative work-site prevention policy. Finally, conditions are identified which either impede or promote work-site prevention.
